All fees are due in FULL upon the RELEASE OF YOUR PET. Applications to defer payment MUST BE made in ADVANCE and all unpaid balances will be subject to a finance charge of 1.5% per month (18% per year) with a minimum fee of $3.00 per month. Attorney’s fees of 25% of principle and interest will be due if your account is placed with an attorney for collection. There is a $30.00 NSF fee for all checks returned by your bank.

I am the owner of the above named animal, and I understand that all reasonable precautions will be taken to assure the safety of the above named animal during its stay at Hood Veterinary Hospital. I will not hold the agents of this hospital responsible if the animal should injure itself, escape, fail to eat, become ill, or expire.
